Scheduling your private assessment is a fairly straight forward process...

A Holistic Iridology Assessment is $120 and includes the details listed on the Assessments page.
Call or text Robin Carey at (615) 969-5539. Include your name, phone and request for a desired assessment date/time.
Robin will connect with you personally to schedule, arrange for a $60 deposit and confirm your next steps.
You may print the "Client Intake & Release Form" below, fill it out and bring with you to the session. Or you may fill it out when you arrive for your appointment.

If you need to reschedule or cancel, the request must be made 24 hours prior to appointment time.

If request is made less than 24 hours prior to your appointment time, a refund is not available.

Otherwise, prepaid fees or deposits may be applied to other appointments.
